Setting up an Appointment

Please text or call 719-345-7110, or email easternsuntcm@gmail.com to set up an appointment. To establish portal access provide your email address, name, and phone number. Once your information is provided, you will be entered into the Patient Portal where you can log in to complete online forms and documents, look over available times, and request appointments.

 24-hour Cancellation Policy. The price of the service FEE WILL BE CHARGED FOR FAILURE TO GIVE A 24 HOUR NOTICE FOR CANCELATIONS.
